What You'll Be Doing

As a Home Assessment RN, you will:

• Conduct in-person assessments in clients' homes

• Evaluate clients' general health, functional abilities, and personal care needs

• Review mobility, safety concerns, and daily living requirements

• Identify changes in condition or care needs

• Document assessment findings accurately and promptly

• Communicate relevant findings and recommendations to the care team

• Help support individualized home care planning
  
  

Qualifications

• Active, unrestricted Florida Registered Nurse (RN) license

• Strong clinical assessment and documentation skills

• Excellent communication and organizational abilities

• Ability to work independently and professionally in clients' homes

• Valid driver's license, reliable transportation, and current auto insurance

• Ability to meet applicable employment and background screening requirements

• Home health, senior care, or community nursing experience preferred
