We're seeking experienced and compassionate Registered Nurse (RN) Hospice Case Managers to join our growing hospice team. Opportunities are available throughout Fairfield and Litchfield Counties, and we'll work with you to assign a territory that best aligns with your location whenever possible.
Our service area includes communities such as New Milford, Sherman, Newtown, Bethel, Brookfield, Ridgefield, Redding, Weston, Wilton, Westport, Norwalk, New Canaan, Darien, Stamford, Greenwich, and surrounding towns.
If you're passionate about delivering exceptional end-of-life care while building meaningful relationships with patients and families, we'd love to hear from you.
As a Hospice RN Case Manager, you'll serve as the primary clinical coordinator for your patient caseload, ensuring compassionate, individualized care while working closely with an interdisciplinary hospice team.
At RVNAhealth, we understand that caring for others begins with caring for our team. As a twelve-time Hearst Media Top Workplaces Award winner, we've built a culture where nurses feel supported, respected, and empowered to provide extraordinary care.
When you join our hospice team, you'll become part of a collaborative, mission-driven organization that values clinical excellence, compassion, and work-life balance.
Hospice nursing offers the unique opportunity to care for the whole person-not just the illness. You'll build meaningful relationships with patients and families, provide comfort during life's most difficult moments, and make an extraordinary difference through compassionate presence, expert symptom management, and unwavering support.
Every day, you'll have the opportunity to bring peace, dignity, and comfort to those who need it most.
