THE ROLE

The Director of Health and Wellness is the lead nurse and clinical leader of Flagstone Senior Living, reporting directly to the Executive Director. As the community’s nurse leader, you hold primary responsibility for the health, safety, and wellbeing of every resident across Assisted Living and Memory Care, and you serve as the connective hub between residents, families, physicians, and the care team.

This isn’t a desk job overseeing staff. You’ll practice nursing alongside Med Technicians and Care Partners as their coach and educator, growing their skills, making sure care plans are understood and carried out with excellence, and keeping everyone informed when a resident’s needs change.

WHAT YOU WILL DO

  • Complete nursing assessments for every new resident at admission and at least quarterly, and build individualized service plans around their needs, preferences, and goals.
  • Coach and upskill Med Technicians and Care Partners as their nurse mentor, through hands-on teaching, real-time feedback, and competency verification.
  • Lead care team meetings and care conferences focused on resident updates, best practices, and family engagement.
  • Serve as the primary nurse contact for changes in condition and care level with families, physicians, the Executive Director, and the RDHW.
  • Oversee medication management, including MAR accuracy, pharmacy coordination, and controlled substance processes.
  • Lead the nursing pre-admission assessment and move-in process through each resident’s 30-day review.
  • Keep the community survey-ready and contribute to quality improvement, incident review, and infection control.
  • Partner with Culinary, Engagement, Maintenance, and the Transition Specialist to deliver coordinated, person-centered care.

WHAT YOU BRING

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) with an active Oregon nursing license in good standing.
  • Associate or Bachelor’s Degree in Nursing strongly preferred; LPN considered based on state regulations and community needs.
  • Minimum three years’ nursing experience in Assisted Living, Alzheimer’s care, or Memory Care preferred.
  • Prior experience as a Residential Care Manager, Clinical Director, nurse manager, or in a risk management capacity preferred.
  • Strong nursing assessment skills and clinical decision-making under time-sensitive conditions.
  • Clear, proactive communication with residents, families, physicians, and the care team.
  • Proficiency with electronic health records and working knowledge of Microsoft Word and Excel.
  • Availability for occasional weekend, evening, or on-call work as needed; ability to lift up to 50 lbs.

PAY AND BENEFITS

  • Annual salary of $95,000 – $115,000
  • $5,000 sign-on bonus
  • Full-time, salaried position
  • Medical, Dental, Vision (full-time employees)
  • 401(k) with employer match
  • Generous PTO
  • Pet insurance
  • Perks at Work
  • Employee Assistance Program
  • Voluntary benefits options

Sign-on bonus eligibility, payout schedule, and terms are confirmed in your offer letter.
