We are currently seeking a skilled and dedicated LPN Homecare Visit Nurse to join our team. In this role, you will have the opportunity to make a meaningful difference in the lives of our patients by providing comprehensive nursing care during home visits. Schedule Monday thru Friday (Hours may vary based on each case)
Responsibilities:
- Conduct home visits to assess patients' health status and provide nursing care as prescribed by the healthcare team.
- Administer medications, treatments, and procedures according to established protocols and physician orders.
- Monitor patients' vital signs, symptoms, and progress, and report any changes or concerns to the appropriate healthcare professionals.
- Educate patients and their families about their conditions, medications, and self-care techniques to promote optimal health outcomes.
- Collaborate with other members of the healthcare team, including physicians, therapists, and social workers, to develop and implement individualized care plans.
- Document all patient interactions, assessments, and interventions accurately and timely in the electronic medical record.
- HHA Supervisory visits to monitor care.
- Wound care
Qualifications:
- Current LPN license in the state of Ohio .
- Minimum of 1 years of nursing experience, preferably in a homecare or similar setting.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a collaborative team.
- Compassionate and patient-centered approach to care.
- Valid driver's license and reliable transportation.
- One year of experience as a home care professional nurse and is competent in performing home care comprehensive assessment
