What You'll Do

As a Chaplain you will provide compassionate spiritual support to hospice patients and their loved ones while serving as an important member of the interdisciplinary care team.

Responsibilities include:

• Conducting spiritual and bereavement assessments and developing individualized plans of care
• Providing spiritual counseling, emotional support, and guidance to patients and families
• Supporting families through grief and bereavement, including follow-up services for up to one year after a patient's passing
• Collaborating with nurses, social workers, physicians, and other team members to ensure holistic patient care
• Educating and consulting with the care team regarding spiritual needs and bereavement support
• Participating in interdisciplinary team meetings and care planning discussions
• Maintaining timely and accurate clinical documentation
• Supporting and guiding spiritual care volunteers as appropriate
• Delivering exceptional service and compassionate care in every patient and family interaction

What You'll Bring

• Graduate of an accredited seminary or school of theology, and/or ordained or commissioned by a recognized faith group
• Clinical Pastoral Education (CPE) preferred
• Experience working with hospice, palliative care, grief support, or end-of-life care strongly preferred
• Ability to provide care in an interfaith environment with respect for diverse cultures, beliefs, and values
• Strong communication, counseling, and relationship-building skills
• Valid driver's license and proof of insurance
