Director of Patient Care Services (DPCS) – Home Health

Are you an experienced Home Health or Hospice RN leader ready for an exciting opportunity to make a difference?

Always Best Care Senior Services is seeking a knowledgeable, motivated Director of Patient Care Services (DPCS) to lead our Medicare-certified, accredited Home Health division serving Solano and Napa Counties.

We're looking for a confident clinical leader who understands regulatory compliance, accreditation, and survey readiness—and enjoys building strong teams and efficient clinical operations.

What You'll Do

• Lead clinical operations and supervise our home health care team.

• Ensure compliance with Medicare Conditions of Participation, California Title 22, and accreditation standards.

• Oversee survey readiness, QAPI, clinical audits, and Plans of Correction.

• Monitor OASIS, clinical documentation, care plans, and quality of patient care.

• Collaborate with the Administrator to strengthen clinical operations and support agency growth.

What We're Looking For

Required:

• Current, unrestricted California RN license.

• Must meet California Title 22, Section 74703 DPCS qualifications:

  • Bachelor's degree or higher in nursing or a health-related field: 3 years of qualifying experience within the past 5 years, including 1 year in supervision or administration; OR

  • RN without a qualifying bachelor's degree: 4 years of qualifying experience within the past 5 years, including 1 year in supervision or administration.

  • Qualifying experience must be in a home health agency, primary care clinic, or health facility.

• Home health or hospice clinical leadership experience.

• Experience with accreditation or regulatory surveys and clinical compliance.

• Strong organizational, communication, and leadership skills.

Preferred:

• Previous DPCS, DON, or Clinical Director experience.

• Experience with ACHC, CHAP, The Joint Commission, or CDPH surveys.

• Medicare home health knowledge, including OASIS and QAPI.

• WellSky Home Health experience.
